Community Access & Social Participation

At Conscious Care Group, we believe everyone deserves the opportunity to be part of their community. Our Community Access supports are designed to help participants get out, connect with others, and enjoy experiences that matters. From everyday outings to exciting group activities, we’ll walk beside the participant to build confidence, friendships, and independence.

Support That’s Built Around You

At CCG, we know that everyone’s journey is unique. That’s why our approach is centred on choice, flexibility, and respect.

Our experienced team works with you to plan supports that fit your participants lifestyle. You decide whether you prefer quiet one-on-one outings or vibrant group activities, and we’ll provide the right support to make it happen.

Your safety and comfort are always our priority, but just as important is making sure you feel empowered, valued, and supported every step of the way.
